Formotic Coaching, twelve weeks. $678, paid once. Or $226 a month for three months, $678 in total.

Formotic Term assessment. $339. A ninety-minute assessment, a written capacity report within five business days, and a video walkthrough.

Term itself is $2,712 in total. The assessment is the only part of Term you buy on this website. If you continue, I will send you a private link for the remaining $2,373, in one payment or monthly.

In-person sessions are arranged directly and are not sold through this website.

All prices are in Canadian dollars and include HST at 13%. The price shown is the total amount charged. There are no additional fees, and no amount is added at checkout.

Formotic is registered for HST. The registration number appears on every invoice.

You receive a Stripe invoice for one time purchases, which carries the HST registration number, the statement that the amount includes HST at 13%, your name, and a description of what you bought. Keep it if you intend to claim the expense.

Payment is by card, Apple Pay, Google Pay or Link. Formotic does not see or store your card details. Payments are processed by Stripe.

There is no direct billing. Formotic does not bill insurers, does not bill WSIB, and does not submit claims on your behalf. Some extended health plans reimburse kinesiology services and some do not. Check with your plan before you buy.

Coaching monthly bills $226 on the day you buy and on the same day of the next two months. Three payments in total.

Term monthly bills $678 a month for four months. If your $339 assessment is already paid, your first payment is $339 and the three that follow are $678 each.

Billing stops on its own. Nothing renews and there is nothing to cancel. You are not enrolled in an ongoing subscription.

If a payment fails, Stripe retries over roughly three weeks and emails you. If it has not cleared by the end of that window the billing plan cancels, and I contact you about the balance outstanding.

Promotional codes are limited in number and carry a stated expiry date. Where a promotion applies to paying in full only, that is stated on the code. One promotional code applies per purchase.

Availability figures on this website are the number of open places on the roster and are updated manually.

One refund exists, and it is this one. If I read your health intake and decide Formotic is not the right service for you, you are refunded in full and I tell you what category of provider to look for instead. That decision happens before anything is delivered.

Beyond that, purchases are not refunded once delivery has begun. Delivery begins at your first scheduled session, or when your program is released to you, whichever comes first.

The $339 is not refunded. It covers half of month one and your report is yours either way. A missed session is not refunded, and one rebook is available at my discretion.

Deciding after your assessment that Term is not for you is a normal outcome and not a refund event. The assessment was delivered and the report is yours.

Discovering that something in your situation sits outside what a kinesiologist does is also not a refund event. I will tell you, I will name the kind of provider you need, and in most cases the physical work continues alongside that care.

Sessions are booked through TidyCal and the cancellation window is shown when you book.

A missed session is not refunded, because the time was held and cannot be filled at short notice. Tell me as early as you can and it is usually a reschedule rather than a miss. One rebook is available at my discretion.

Injury, illness, a medical emergency or significant hardship pauses your cycle rather than ending it, for up to ninety days. On monthly payments, billing pauses too. Tell me and it is arranged.

Stopping ends future delivery. Instalments already taken are not returned, and instalments for a period already delivered remain payable.

Nothing in these terms limits any right you have under the Consumer Protection Act, 2002 (Ontario) or other applicable consumer protection legislation.

Buying is one step. The service runs on a written agreement, which I send you for signature the same day you pay.

That agreement covers scope, health information, what your capacity report is and is not, emergencies, and the limits of what I do. Read it rather than scrolling to the bottom.

Where these terms and your signed agreement touch the same subject, the signed agreement prevails. These terms cover the purchase. The agreement covers the work.

Delivery does not begin until the agreement is signed by both of us. If you buy and then decide not to sign, tell me and you are refunded in full.

Your health intake, your video, your logged training data and your capacity report are personal health information and are handled under the Personal Health Information Protection Act, 2004 (Ontario).

No health information is collected at checkout. Stripe holds your name, billing address, phone number and payment record, and nothing about your body or your history. Health information is collected separately through the intake form after you buy, and it is described in your agreement.

Your footage and your data are never used publicly or promotionally without your written permission at the time.

Formotic delivers kinesiology and strength and conditioning services. It is not medical care, physiotherapy, chiropractic, psychotherapy, dietetics or nutrition counselling.

Your capacity report is a training document. It is not a functional capacity evaluation, it is not prepared to any adjudicative standard, and it cannot be used in a claim, a hearing or an independent medical examination.

If another provider has given you restrictions, their instructions take precedence and I program around them.

Nothing on this website is a guarantee of a specific result.

These terms may change. The version that applies to your purchase is the version published on the date you bought, and the last updated date is at the top of this page. Changes are not applied backwards.
